To the Parents/Guardians of incoming sixth grade students:
NJ State Department of Health and Department of Education immunization requirements for students entering 6th grade in September are:
Diphtheria, Tetanus, and Acellular Pertussis booster (Tdap) and Meningococcal vaccines.
If you child has already received these immunizations, please submit records to the Health Office of your child’s current 5th grade school in the spring. If your child receives immunizations in the summer after the end of 5th grade, you will need to forward immunization records to the Mount Olive Middle School. The immunization record must be an official copy issued by you health care provider.
If you have questions about these vaccines, please contact your healthcare provider. If you do not have a healthcare provider, you can contact the Mt. Olive Board of Health for free or reduced cost immunizations. The Health Dept. can be reached at (973) 691-0900 x7353 to schedule an appointment.
All immunization records must be submitted no later than September 1st. If your child turns 11 years old after September 1st , you will have 14 days from the birth date to submit documentation. Please be aware that if requirements are not met, your child will be excluded from school as per NJ State Administrative Code 16.
